PAYMENT INSTRUCCIONS:
- To make a PAYMENT or DEPOSIT type on the first option (next to Dr. Sandy Martinez) the word "DEPOSIT" if applicable or just the word "PAYMENT" Typing any other word might result on payment delay.
- On the second option just write the amount in US dollars.
- Select the form of payment you wish to use like PayPal or any mayor credit or debit card.

IMPORTANT NOTES REGARDING DEPOSITS AND PAYMENTS:
- Your deposit is NON refundable for any reason however you can change the date and YOUR DEPOSIT WILL NEVER EXPIRE.
- If you make a full payment only the portion of your deposit is commission free the balance will incur a 4.9% transaction fee.
- If you need financing please contact your patient services manager to apply.
